APCC PROGRAM

Diabetes Management and Prevention

Aims

Provide skills, tools, strategies and support to improve on goal alignment, service gap identification, responsiveness and effectiveness in provision, delivery and coordination of diabetes health care services

Structure

  • Two face to face training sessions:

Saturday 26 Nov 2011 and 21st April 2012 (9am-5pm)

Venue:  Four Points Sheraton Hotel Darling Harbour

  • Two online training sessions in February and June 2012
  • Monthly one on one consultancy sessions and PDSA cycles submission

Performance Indicators

Diabetes register: number of patients diagnosed/coded
with Diabetes); HbA1c<=7% (percentage of patients meeting this criteria); AUSDRISK tool (percentage of eligible patients without diabetes and with an AUSDRISK tool performed);

Diabetes Annual Cycle of Care (elements of the cycle
recorded for patients in the diabetes register)

A selected group of practices are currently participating in the APCC (Australian Primary Care Collaboratives) program which is the largest Primary Care Improvement program in Australia. Approximately 1100 practices have participated nationally in the APCC program.

 

The APCC is about improving outcomes for patients with a chronic disease, collaborating and sharing ideas that work and monitor if the practice is on the right track to succeed.

A collaborative is a strategy for achieving rapid improvement in clinical outcomes.
This is achieved by
using small cycles
called PDSAs
(Plan Do, Study, Act)
.

Participant practices
apply PDSA cycles to sustain change in small incremental steps. The PDSA cycle allows users to test and implement solutions and processes at a small scale before rolling these out to the
rest of the practice. 

The current program is specific to diabetes. Practices have achieved improvements in:

Setting up systematic processes by which diabetes reviews and plans are accomplished e.g. checklists. Promotion and maintenance of correct coding.
Identification of patients eligible for AUSDRISK tool health assessment. Patient identification for referrals.
Structured completion of clinical data for diabetic patients in the practice. Use of the RACGP Primary Care Sidebar© to maximise efficiency in caring for patients missing information and care (Red flag) particularly for Diabetic patients.
Patient education on self blood sugar monitoring. Running Diabetes clinics supported by the Pen CAT Tool.

Practices can expect to:

  • Improve the health of patients;
  • Enhance practice clinical reporting and functionality;
  • Improve data quality for patients with Diabetes;
  • Build a stronger practice team and improve practice efficiency and performance;
  • Enhance quality improvement skills of practice staff;
  • Receive a financial practice incentive to participate;
  • Receive professional development points for GPs and Practice Nurses.
