Lifestyle Modification Program (LMP)

WentWest's Lifestyle Modification Program (LMP) is now run through SHAPE - an 8 session dietary education and exercise service for at-risk patients.

To be eligible, patients must meet the programs entry criteria:

  • 40-49 years and high-risk of developing Type 2 diabetes according to AUSDRISK assessment;

    OR

  • 15-54 years Aboriginal or Torres Strait Islander at high-risk of developing Type 2 diabetes according to AUSDRISK tool.

During the meeting, we will discuss how to formulate the education module with priority given to areas of greatest community health needs. We will also look into the methodology of delivering the monthly educational meetings to participants at all levels.
