Lifestyle Modification Program (LMP)
WentWest's Lifestyle Modification Program (LMP) is now run through SHAPE - an 8 session dietary education and exercise service for at-risk patients.
To be eligible, patients must meet the programs entry criteria:
- 40-49 years and high-risk of developing Type 2 diabetes according to AUSDRISK assessment;
OR
- 15-54 years Aboriginal or Torres Strait Islander at high-risk of developing Type 2 diabetes according to AUSDRISK tool.
